Case Management Services

Case management is a service covered by Medicaid and provided by Carousel Pediatrics. Our case managers are licensed social workers or nurses who assist children, women who are pregnant, and their families with:

Access to needed medical services
Family problems
Education/school issues
Financial concerns
Finding help near where they live
Equipment and supplies

What do Carousel’s case managers do?

Carousel’s case managers empower patients and their families through personal interaction and dedication to:

Find out what families need,
Make plans to meet those needs,
Help families find the necessary services near where they live,
Refer children, women who are pregnant, and their families to community resources and other providers,
Teach individuals and families how to find and get services they need, and
Follow-up with families to ensure those needs have been met.
