Case Management Services
Case management is a service covered by Medicaid and provided by Carousel Pediatrics. Our case managers are licensed social workers or nurses who assist children, women who are pregnant, and their families with:
- Access to needed medical services
- Family problems
- Education/school issues
- Financial concerns
- Finding help near where they live
- Equipment and supplies
What do Carousel’s case managers do?
Carousel’s case managers empower patients and their families through personal interaction and dedication to:
- Find out what families need,
- Make plans to meet those needs,
- Help families find the necessary services near where they live,
- Refer children, women who are pregnant, and their families to community resources and other providers,
- Teach individuals and families how to find and get services they need, and
- Follow-up with families to ensure those needs have been met.
